GHM Application Document: 


Thank you for applying for GHM membership. Please check the list below and email all relevant documents to the Organizing Committee of Global Health Forum of Boao Forum for Asia before submitting your application.


1. Application form - stamped and signed by the legal representative of the applicant enterprise/organization;

2. Representation - stamped and signed by the legal representative of the applicant enterprise/organization and with the official seal;

3. Logo of the applicant enterprise/organization (one Illustrator vector trademark file including one original color trademark in PNG format, one reverse white trademark in PNG format and one ink draft trademark in PNG format);


4. 1 or 2 photos (in JPEG or PNG format, under 3Mb) of the chairman, CEO or president of the applicant company/organization (no more than 3 corporate leaders).

 

Note:

The applicant company/organization must complete the application form and email the electronic version of other requested materials and the scanned copy of stamped  documents to the organizing committee at GHF@boaoforum.org


The original application documents mentioned above should be delivered to the Organizing Committee of Global Health Forum of Boao Forum for Asia.
